Passports

1. All applicants must appear in person, including both parents of children under age 16 and one parent for applicants who are age 16-17. 
   
   
2. The application must be typed and/or printed using BLACK ink prior to appointment. 
   
   
3. Proof of U.S. citizenship is required via an original/certified U.S. birth certificate, original certificate of naturalization, or previous passport. 
   
   
4. Proof of parental relationship (original/certified birth certificate) is required for all applicants 17 years of age and under. 
   
   
5. Original documents must accompany application to the U.S. Department of State. 
   
   
6. Two checks are required; one for U.S. Department of State and one for City of Walnut. 
   
   
7.  You must establish your citizenship to the satisfaction of the acceptance agent and Passport Services. We may ask you to provide additional evidence to establish your claim to U.S. Citizenship. Visit travel.state.gov for details. 
   
   
8. One color photograph (2 x 2 inches). Passport photo services now available onsite for a fee of $17.
   
   NOTE: Along with ORIGINAL documents, you must also submit a single-sided COPY of each document with your application.

Passport Processing Fees

Please note: The City of Walnut only processes DS-11 forms.

Payment Instructions: The City of Walnut requires two (2) separate checks for passport processing. NO credit cards or debit cards are accepted. The application fee is paid directly to the Department of State (DoS). The Execution/Acceptance fee is paid directly to the City of Walnut. Please note that passport photo services incur an additional $17.

Passport Cards: Passport cards are valid only for travel by land and by sea to Canada, Mexico, Bermuda, and the Caribbean, not for international air travel.

ADULT APPLICANTS (16 Years and Older)

OPTIONAL FEES (Paid to the U.S. Department of State)

• Expedited Fee ($60) - Paid per application, in addition to required fees. Provides faster processing than routine service.
• 1-2 Day Delivery ($22.05) - Paid per application for 1–2-day delivery of issued passport book from the Department of State to customer.Only applies to mailing addresses within United States, not valid for passport cards.
• File Search Fee ($150) - When applicant is unable to present evidence of U.S. citizenship or verification of previously issued U.S.Passport/Consular Report of Birth Abroad - submit Form DS-11 and written request for file search.

The City of Walnut is now proud to offer passport photo services! Photos can be taken and printed onsite for $17. When booking an appointment, please be sure to indicate that you are in need of photo services.
